Got me there. I'm so used to that never changing, I forgot it had other
meanings.
Ok, I found this list concerning that first digit:

0: regular UPC codes
1: reserved
2: random weight items marked at the shop
3: National Drug Code and National Health Related Items code
4: no format restrictions; for in-shop use on non-food items
5: for use on coupons
6: reserved
7: regular UPC codes
8: reserved
9: reserved

But, I'm not sure how that digit fits into the scheme.
If your individual items are 3:National Drug/National Health Related, why
would the case not be marked with the 3 as well, as it is the same product?

Nope - the same manufacturer. The "58970" part identifies the
manufacturer in this particular coding scheme. The leading "3" in the
consumer code identifies it as an NDC code (or something to that affect).
