Yes, there are probably *scores* of EDI management systems that would
satisfy your requirements if you want to interface using flat files as an
intermediate format for importing/exporting data to/from your database. If
you want to set up a direct link from the EDI system to the database, a few
of the standard EDI packages support SQL or ODBC compliant databases as
data sources and targets rather than requiring you to use flat files.

There are a few API libraries that are advertised to handle EDI directly
and provide a programmatic link to your application. I'm sure their
vendors would be glad to contact you off-line.

As to your other questions:

"How to prepare the EDI files with our data and how
they reach to the insurance company ? "

You'll need to HIPAA guidelines, and some help - at least in the form of
subscribing to the WEDI SNIP listserv (see other messages if I've not
gotten that correct).

"How the Hospitals get back their money from the insurance
companies, how they handle it ? "

Generally the same answer, though this gets into things like doing
electronic funds transfer versus paper checks, and other business issues
which are usually beyond the scope of this list.
